It seems I'm still getting some qualtiy mis-match issues when I re-smelt ingots together.
This seems to be the case when I smelt two of the same together and their output is the same quality of ingot. Example:
I have 4 low-grade ingots of silver that VT all show as identical quality. However, I go to smelt two together, yield a low-grade ingot of silver and it ends up being a different quality than the rest in my inventory.

Re: Re-smelting and Quality Mismatch
I haven't seen it again. I'm thinking it might be because I was using older metal (metal that was smelted with the new changes but before the fix). I thought I had used it up, but it's still possible that I had some left.
Just wanted to give a head's up in trying to knock this out.

Thanks Athon, any more clues you have might help, I thought I had knocked this for six....about 5 times!

Re: Re-smelting and Quality Mismatch
I tested this on the testport, and was able to reproduce it with old metal, but had no troubles after munching all my old metal and mining new stuff. Moving to resolved.
