I might be martyring myself for this:
When a person is starving, they tend to lose their muscle density as well as concentration.
All that starvation or thirst do in the game is dock you a few hp and then you get them back in about 3-4 rounds.
Perhaps either your:
Concentration checks are lowered
Constitution is lowered
Strength is lowered
Health is severely lowered
Mental health is lowered
Etc.
Lord save us from a lawyers ectetera.

Already when you are starving/thirsty, your mental health suffers. As well, your stats do not recover at nearly as high a rate as if you were not starving/thirsty.
But really, I don't much like this idea. I've noticed that most of the times when a character starves or becomes thirsty is in the middle of an RP. They do not eat because it is very unIC to whip out a plate of shishkabob in the middle of a heated situation, or a ceremony, ect. In good RPs that end up in PKill situations, I'd rather not punish players with lowered strength because they took their time to RP things out thoroughly and properly.
Your suggestion is very realistic. But I don't feel that more realistic is necessarily more fun.

You can go weeks without food. Itll take some considerable time for your body to break down to reach any state where your strength and health deteriorate to any degree of uselessness. If anything itll be the madness of starvation that would affect you the most and the worst. RPable? Yes. Codeable? Doubt its even worth it.

Conversation to stave off hunger?
This brings up a related point that I've oft wondered about - would it be possible/reasonable to have one's hunger not increase if one is in the presence of lots of say/sayto/smote's? Then, when one is in the midst of a conversation, the urge to whip out said sishkabob doesn't develop? (Similarly with the need to slake one's thirst)
Not sure how much of a coding nightmare that would be; might well not be worthwhile. With all the grand code improvements made of late, I feel really greedy even suggesting this, but the topic has come up.
