A medium green beetle quickly turns to avoid your sneak attack!
After finishing a stunned beetle, I attacked the second one, as you can see it does not show "You surprise a beetle etc." as in a normal backstab, yet the beetle avoids a sneak attack.

Before it occurred that on a regular backstab, started by attacking from hidden, the beetle received two backstabs in a row, despite my thief is still far from having second attack.

Backstab applies any time an opponent is denied their dexterity bonus to AC, which includes on the first round of combat. When you get a "Double backstab", one is from your surprise round while the other is you winning initiative on the first 'actual' round of combat.
What are you talking about? What, that guy?
That was like that when I got here.
