I have run into this situation again. I did some testing to see if I could narrow down the issue. I checked various times and conditions. Here is what I found. I hope it helps.
Turning in a scalp at 5am resulted in no damage

I realized one thing I had in common every time I took damage was that I was hurt before I started giving scalps so I healed myself and tried again. I found once I had no damage on me I stopped taking damage from the scalps. This was still at 9am.

Is it possible that having damage or having a specific amount of damage on a specific body part is contributing to this issue?
