He is bleeding freely.
I wasn't fighting anything, to my knowledge, with a specially-set bleeding weapon (just in Howling Peaks).
I was down to 25% when it started happening. You don't become unstable in D&D until after you reach 0 hp. In FK it normally doesn't happen until after you've lost a limb.

I don't want to be unappreciative of your taking the time to report a bug, but I also have way more bug reports than I have time to tackle - and this one is just too mushy to offer much hope on its own.
If others have experienced bleeding wounds when they think they should not, please report it here.
Is anyone certain that "bleeding heavily" shouldn't occur save when hit by an enchanted weapon or you lose a limb? That's not obvious to me at all.

Any weapon, even fists should cause bleeding at low health in theory.
Justice is not neccesarily honourable, it is a tolerable business, in essence you tolerate honour until it impedes justice, then you do what is right.
