I recently GM'd the 'mount' skill with Garumsh, and have been noticing an odd number of failures that I do not remember occurring in the past. Among these include a great deal of falling off during combat (I have the mounted combat feat) and actually failing to use the mount command.

I suddenly found I had the same issue but then I realized my mount wasn't wearing its saddle. I don't know if this has anything to do with your issue.

That seems to have been the issue. I didn't realize saddles had any coded benefit, and it didnt make sense to me to put one on a worg.

One upon a time you could ride without a saddle if you were a grandmaster of mount, that changed a long time ago. It makes more sense to use a saddle, anything else is painful.
Justice is not neccesarily honourable, it is a tolerable business, in essence you tolerate honour until it impedes justice, then you do what is right.

I believe that there ought to be only a very small, if any, chance of a grandmaster of mount falling off his mount, with or without a saddle.

Anyone that good at riding is not going to fall off while walking down a road, regardless of whether or not they are sitting on a saddle. Perhaps speed could play a factor in how likely a GM is to fall off, but at walking speed I would say it would never happen.

Can anyone answer the other half of this? Are grandmasters at mount still falling off their steeds when moving at a walk on roads?
(I think the steed may also make a difference?)

No, seemingly the problem has been resolved. Very few, if any, falls are evident even while moving as fast as "soar" while flying, and while "gallop"ing while on land.
