I found what I believe to be two bugs related to attacks of opportunity in a single fight with "a small, disfigured wererat."

The first is that I received an attack of opportunity each time my opponent attacked me.

You attack a small, disfigured wererat as he gets within your weapon's reach.
Your slash minces a small, disfigured wererat's right leg.
A small, disfigured wererat's punch brutally pulverizes your left arm.

It looks like this particular were rat is attacking with punches instead of natural attacks. I believe wererats are supposed to use claw and bite attacks, correct?

The second bug I came across happened when my torch went out. I was no longer able to see my enemy, but he still provoked attacks of opportunity from me.

You attack someone as he gets within your weapon's reach.

It is to my understanding that you do not get attacks of opportunity against foes that you cannot see or otherwise locate.

You are right about the attack type of the wererat: the description and greet echo suggest is in rat form. It is set as race human, perhaps because the lycanthrope race would be too powerful for low level PCs, or contagious? Area gurus have any insights on that? Could we make it race rat?

The attacks of opportunity you saw here result from this rule:

I don't know if you are still supposed to get this attack if it is dark and you can not see, but it would make sense to me you still might. The attacks of opportunity are provoked by the wererat fighting as "unarmed," so changing it to a rat with claw attacks should remedy the problem.
