On killmode stun, chain lightning (at least) seems to be causing enough damage to make the victim bleed to death.
On killmode stun, shockshield (at least) seems to be nullifying the "stun" effect, allowing me to keep attacking for a while after being stunned as long as someone is trying to hit me. On two occasions this has led to me being reduced to -5% hp and subsequently dying.
I wouldn't be surprised if other spells suffer the same side effects, but I haven't done enough thorough testing to know for sure.

Odd. I tried this with a level 44 illusionist, GM in chain lightning, on four small brown and black rats. No dice! They were all stunned, non bleeding, none died. How can I make this better emulate your case? However, that said, it is certainly possible with some other skills and feats, and has come up before. Should stun be a solid guaranty that someone you have attacked will not die at your hand; that is, the bleeding flag should never be set while you are in killmode stun?

If you keep casting chain lightning after the person is stunned, it seems to cause bleeding out.
It may have been a side effect of being grouped with someone else who wasn't stunned. In all the cases I can recall, the victim was grouped with someone else (most recently, myself) and bled out as a result of chain lightning spells that were cast on me (while I was still standing) and arced over to them.
I have seen this occur in multiple cases, so I think it's reproducible.
Personally, I think stun is intended to be non-lethal for sparring, so allowing bleedout sort of defeats the purpose. But if someone disagrees we can open it up for discussion.
