I know you're going to redact most of this, but I need to include it so you know where I'm speaking of.

I entered the "On a raised dais" from the western room. I'm unable to go back west?

You enter "The bottom of a hole" from there you go west to "An underground river" which then continues to flow west to "An underground lake". This room has a description that includes

"The water seems to come from a flooded
tunnel to the east and to flow, very slowly, towards the
south, going through a crack in the stone far too small for you"

That appears to me to be a broken exit.

There was a bug in a program on the first room you mentioned.

The second room you mentioned is not broken. That comment is just part of the room's description. There is not an exit there.
