[Feat] Combat Casting

While reading up on concentration on d20srd.org I came across this passage:

I checked the mud helpfile:
help combat casting wrote:The combat casting feat enables you to concentrate better when casting spells in combat. This feat has no prerequisites.
Does that mean that the combat casting feat only gives a bonus to concentration checks when on combatmode +defensive or while grappled by a spell? (I'm not sure about the latter case, since I've never seen a "grapple" type spell that allows one to cast while held)

Hviti's question can be extended to almost all feats in the game whose help files are ambiguous at best.

It would be helpful if these were explained in a manner that people know what they're really getting for their feats.
